چکیده
E nveloped in poverty and burdened by its onerous topography, Nepal is hardly an obvious birthplace for a public health success story. And yet, the marriage of scientific and operational innovations provided the foundation for a national program that saves hundreds of thousands of children’s lives. In the early 1980s, a series of studies demonstrated that vitamin A supplementation could reduce child mortality; two of them were carried out in Nepal.1,2 Following this discovery, Nepal conceived of the National Vitamin A Program to distribute small, red vitamin A capsules to millions of children each year. Its creative design overcame the country’s economic and geographic hurdles by harnessing local capacity and relying on new channels of public health delivery—bridging the once insurmountable gap between where the formal health sector Case drafted by Jessica Gottlieb. The impact of vitamin A supplementation on mortality inequalities among children in Nepal.
OBJECTIVE This paper examines gender, caste and economic differentials in child mortality in the context of a cluster-randomized trial of vitamin A distribution, in order to determine whether or not the intervention narrowed these differentials. DESIGN The study involved secondary analysis of data from a placebo-controlled randomized field trial of vitamin A supplements. The study took place ...
متن کاملMortality of infants < 6 mo of age supplemented with vitamin A: a randomized, double-masked trial in Nepal.
The effect of supplementing 11,918 infants < 1 mo and 1-5 mo of age with vitamin A (15,000 and 30,000 micrograms retinol equivalents or 50,000 and 100,000 IU, respectively) or a placebo on subsequent 4-mo mortality was assessed in a randomized, double-masked community trial in the rural plains of Nepal. There were 130 deaths (51.6/1000 child-y) in the control group and 150 deaths (57.1/1000 chi...
متن کاملNeonatal health in Nepal: analysis of absolute and relative inequalities and impact of current efforts to reduce neonatal mortality
BACKGROUND Nepal has made substantial progress in reducing under-five mortality and is on track to achieve Millennium Development Goal 4, but advances in neonatal health are less encouraging. The objectives of this study were to assess relative and absolute inequalities in neonatal mortality over time, and to review experience with major programs to promote neonatal health. METHODS Using four...

متن کاملAntenatal and postnatal iron supplementation and childhood mortality in rural Nepal: a prospective follow-up in a randomized, controlled community trial.
The long-term benefits of antenatal iron supplementation in child survival are not known. In 1999-2001, 4,926 pregnant women in rural Nepal participated in a cluster-randomized, double-masked, controlled trial involving 4 alternative combinations of micronutrient supplements, each containing vitamin A. The authors examined the impact on birth weight and early infant mortality in comparison with...
